About
Detected need
There are and will be more and more people in the process of ageing and with needs for personal care at home, care and monitoring and support from health and welfare services. The current lack of coordination between health facilities, services and home care entities causes duplication, prevents preventive action and coordination with the Primary Care Centre. It is necessary to optimise devices and resources and new strategies, to guarantee the right to independent living and personal autonomy, always placing the person at the centre of the decisions to be made. This is possible by innovating through processes based on personalised care based on value. It is now possible to advance in the solution by devising an effective system of relationship and coordination between home service providers and health professionals.
Proposed solution:
Creation of a Home Care Network made up of the set of operators, entities and actors involved in home care for the elderly and/or those in a situation of dependency. The aim is to share information and best practices, improve monitoring and care through a coordinated strategy between services that do not currently communicate with each other in the appropriate way, and in this way prevent hospitalisations and delay or avoid the institutionalisation of the people cared for, guaranteeing their right to independent living and personal autonomy. With the Network and a simple digital platform, the aim is to improve the response capacity of people and care professionals, making it agile and effective in situations of risk or deterioration in health and towards the primary care centre. Likewise, the aim is to empower the people cared for and improve the skills and training of caregivers and professionals, who in the project will share workshops and sessions to share good practices. The project is being developed by a multidisciplinary team based on a co-creative methodology aligned with the principles of Person-Centered Care (PCC) and Value-Based Health Care.
